Please screen yourself for COVID-19 symptoms the morning of your appointment using the COVID-19 Checklist below.<\/p>\n

We will be asking you to confirm you are healthy and do not have or any household members have or have had COVID-19 symptoms. COVID-19 Testing:\u00a0 If a customer, or anyone in their household, has recently had a COVID-19 test, the customer may not visit any Pet Pawdicure location until test results are known and are negative.<\/p>\n

  1. Do you have any COVID-19 positive members in your household or close contacts?<\/li>\n
  2. Have you had close contact with someone waiting for test results for COVID-19?<\/li>\n
  3. Have you had, tested positive for, or are awaiting test results for COVID-19?<\/li>\n
  4. In the last two weeks, have you worked or volunteered in a healthcare facility, or as a first responder?<\/li>\n
  5. Are you feeling ill? Have you been in contact with someone who is ill?<\/li>\n
  6. Have you had domestic or foreign air travel within the last 30 days?<\/li>\n
  7. Have you visited any \u201chot spots\u201d in the past 30 days?<\/li>\n
  8. Do you have any of the following symptoms of COVID-19?:\n

    1. Cough, shortness of breath, mild or moderate difficulty breathing<\/li>\n
    2. Fever over 100 degrees or feeling feverish (chills, sweating)<\/li>\n
    3. Sore throat, have a cold, sneezing, congestion or runny nose<\/li>\n
    4. Feeling under the weather, fatigue; muscle or body aches<\/li>\n
    5. Vomiting or diarrhea<\/li>\n
